Subject Access Request (SAR) Form

In data protection law (Article 15 of the UK GDPR), people have a ‘right of access’ to their own personal information. This means they can request any personal information that SEUPB may hold about them, known as a subject access request (SAR).

One way to submit a subject access request to the SEUPB, is to make your request to the SEUPB’s Information & Research Officer through the form below.

Notes:

  1. In order to verify your request for personal data, you may need to provide us with adequate Proof of Identity. 
  2. Normally, information will be supplied free of charge, but in certain circumstances a fee may apply. Where a request is manifestly unfounded, excessive, of a repetitive nature or where more than one copy of the data is sought, a fee may apply. 
  3. You may contact our Data Protection Officer at dataprotection@seupb.eu to assist you in the completion of this Form.

  • Birth certificate and I.D in the case of a parent/child
